Using emotions to cast light into the “black-box” of user adaptation and continued use of health information system

Although emotions are often evoked when medical professionals are required to employ new systems at work, their roles in information system post-adoption literature are relatively less examined. We draw on appraisal theories of emotions to unveil the emotional mechanisms underlying medical professionals' behaviors at different post-adoption stages. Results from data collected from a two-round survey demonstrate the roles of emotions such as irritation, interest, and satisfaction in explaining user adaptation at the initial post-adoption stage and the role of emotions such as pride and satisfaction in determining the continued use of technology following user adaptation at the later post-adoption stage.

Nurses' Informatics Competency Assessment of Health Information System Usage: A Cross-sectional Survey.

Nurses' informatics competencies are nurses' professional requirements to guarantee the quality of patient care and affect nurses' use of health information systems. The purpose of this survey was to describe nurses' perceptions of their informatics competencies regarding health information system usage. A previously tested web-based questionnaire with multiple-choice questions was sent to nurses whose e-mail address was available through three Finnish Nursing Associations (N = 58 276). A total of 3610 nurses working in Finland responded. Both descriptive and explanatory statistics were used to analyze the data. The three dependent variables "nursing documentation," "digital environment," and "ethics and data protection" were formulated from the data. Nurses' overall informatics competency was good. The "ethics and data protection" competency score was higher than that of "nursing documentation" or "digital environment." Recently graduated nurses and nurses working in outpatient care, virtual hospital, examination, or operation had highest "digital environment" competency score. Health information system experience was associated with "nursing documentation." Nurses are highly qualified health information systems users. However, the competency requirements generated by rapidly expanding digitalization have challenged nurses. It is important to increase educational programs for nurses of how to use digital devices, and how to support patients to use digital services.

Investigation of eye movement measures of mental workload in healthcare: Can pupil dilations reflect fatigue or overload when it comes to health information system use?

The use of health information systems (HIS) can result in high workloads and, consequently, poor performance characterized by e.g. increased occurrence of errors among clinicians. Pupillometry offers a good possibility to measure mental workload in a dynamic work setting. Currently, there is a lack of empirical research in the context of healthcare settings. Therefore, the aim of the present study was to examine whether specific eye movement measures are suitable for measuring mental workload in the healthcare setting, especially when working with HIS. 49 persons participated in our simulation-lab study. They had to complete a system-related task as well as an increasing n-back task. Both tasks were modified regarding task difficulty. Results show significant differences for objective and subjective workload measures between increasing task levels. There are also hints for an overload/fatigue indicator in pupil data. Our results are limited in terms of external validity, causality and effects. Future work should focus on high-fidelity simulations and less time-consuming analysis approaches.

Cognitive Ergonomics: A Review of Interventions for Outpatient Practice.

Doctoring is difficult mental work, involving many cognitively demanding processes such as diagnosing, decision-making, parallel processing, communicating, and managing the emotions of others. According to cognitive load theory (CLT), working memory is a limited cognitive resource that can support a finite amount of cognitive load. While the intrinsic cognitive load is the innate load associated with a task, the extraneous load is generated by inefficiency or suboptimal work conditions. Causes of extraneous cognitive load in healthcare include inefficiency, distractions, interruptions, multitasking, stress, poor communication, conflict, and incivility. High levels of cognitive load are associated with impaired function and an increased risk of burnout among physicians. Cognitive ergonomics is the branch of human factors and ergonomics (HFE) focused on supporting the cognitive processes of individuals within a system. In health care, where the cognitive burden on physicians is high, cognitive ergonomics can establish practices and systems that decrease extraneous cognitive load and support pertinent cognitive processes. In this review, we present cognitive ergonomics as a useful framework for conceptualizing an oft-overlooked dimension oflaborand apply theory to practice by summarizing evidence-based cognitive ergonomics interventions for outpatient care settings. Our proposed interventions are structured within four general recommendations: 1. minimize distractions, interruptions, and multitasking; 2. optimize the use of the electronic health record (EHR); 3. optimize the use of health information systems (HIS); and 4. support good communication and teamwork. Best practices in cognitive ergonomics can benefit patients, minimize practice inefficiency, and support physician career longevity.

Barriers, facilitators and motivators of electronic community health information system use among health workers in Ethiopia.

The electronic community health information system (eCHIS) has been implemented in Ethiopia to support health services delivered by community health workers. Despite the many benefits of digitizing community health information systems, the implementation of the eCHIS is challenged by many barriers resulting in low uptake. This study assessed the barriers, facilitators, and motivators of eCHIS use among health workers with focus on health extension workers (HEWs) in Ethiopia. Phenomenological approach was used to assess the barriers, facilitators and motivators of eCHIS use in Amhara, Harari, Oromia, Sidama, South West Ethiopia and Southern Nation Nationalities and People's regions of Ethiopia. Data were collected from 15-29 May 2022. A total of 54 face-to-face in-depth interviews were conducted among HEWs, HEW supervisors, health information technicians and managers. The interviews were audiotaped using Open Data Kit, transcribed verbatim and translated into English. OpenCode 4.03 software was used for coding and categorizing the data. Thematic analysis was used to analyze the data. The HEWs and other eCHIS users reported lack of infrastructure and resources; poor quality of training, follow-up, and supervision; parallel recording using the manual and electronic system; and HEWs' workload as barriers hindering eCHIS use. Data quality, retrievability, and traceability; tablet portability; encouragement from supervisors; and positive image in the community resulting from HEWs using tablets in their routine activities were the main facilitators of eCHIS use. The study identified various barriers that adversely affect the use of eCHIS. An integrated and coordinated approach to eCHIS implementation that encompasses removing the barriers, and reinforcing facilitators is required.

Analysis of Barriers to the Use of Health Information Systems for Tuberculosis Patients at the Parongpong Community Health Center, West Bandung Regency, Indonesia

Based on the rules from the government contained in the Regulation of the Minister of Health of the Republic of Indonesia Number 31 of 2019, the Public Health Center information system can be implemented electronically so that the data obtained can be stored safely. An electronic health center information system must have an application, an internet, and a local network (LAN). This can also support Health Promotion activities through electronic media and online reporting to the Minister of Health of the Republic of Indonesia. This research aims to apply innovative health education to the problem of low Tuberculosis patient visits. This is done by analyzing a fishbone diagram and determining priority solutions to problems that have resulted in no real follow-up plans (RTL) so that targets have not been met and there has been no evaluation action at the Parongpong Health Center. This activity is carried out in several stages, namely 1) problem analysis with fishbone, 2) USG (urgency, seriousness, growth), and 3) SWOT analysis. Furthermore, counseling was carried out to all people in the Parongpong sub-district. The results obtained from calculating the score using the SWOT analysis are to make regulations on procedures and standard operating procedures (SOP) for the use of the Tuberculosis Information System (SITB) to optimize health information management at the Parongpong Health Center, West Bandung Regency, Indonesia.

The impact of EU Digital Services Act and Digital Markets Act on health information systems

The European Parliament has approved new legislation, the Digital Services Act (DSA) and Digital Markets Act (DMA) to improve the functioning of the internal market of intermediary services in the European Union (EU) where there is a risk that the major so-called gatekeeper companies can exercise unfair control of core platform services. The purpose of this study was to investigate, what health information systems could be in the scopes of these acts and what requirements the acts may have for the production, the sale and the use of health information systems. The act texts were examined bearing in mind what types of health information systems exist and what their user bases are. Those health information systems that can belong or do not belong to the groups of systems regulated by the DMA and DSA were identified. The most relevant requirements for these systems were also identified from these acts. The result of the study is that these acts have only minor consequences for the healthcare information systems sector as they are not often intermediary (hosting) services in the meaning of the DSA or gatekeepers in the meaning of the DMA. The emerging digital healthcare platforms are most affected by the new DSA and secondly such peer support patient portals where patients can supply content for others to see. Apparently, no digital healthcare platform has yet reached such a size or a dominant role within the EU that it would fall under the scope of the DMA. The two above mentioned healthcare related intermediary services have due diligence obligations to remove illegal contents from their services and to treat their business and consumer customers fairly. The obligations include clear and fair terms and conditions, the provision of a single point of contact for users and authorities, content moderation, complaint handling, marking advertising clearly, annual reporting, and responding to the contacts from the authorities. The obligations increase when the size of the enterprise increases. It is still too early to produce healthcare information systems specific guidance to support the implementation of these two acts as the acts themselves and potential upcoming general guidance documents can serve the health information systems community sufficiently well.

The health information system (HIS) integrates data collection, processing, reporting, and use of the information useful to measure and assess health and social care for improving their management. HIS has great potential to reduce healthcare costs and improve outcomes. Information may be used to plan community-based care interventions by identifying populations at risk, especially by community healthcare professionals (e.g., family/community nurses). In Italy, HIS collects health and social information of individuals cared by the National Health Service. This paper has two main objectives: (i) to provide an overview of the main existing health and social HIS Italian databases; (ii) to describe the experiences of use of the information of HIS databases in the Piedmont Region.

Health Information System Users in Public Health Facilities: A Descriptive Analytics

Health Information Systems (HIS) are vital in making or developing the policies of health programs in the Philippines. The HIS is broadly defined as a system that integrates data collection, processing, reporting, and use of the information necessary for improving health service effectiveness and efficiency through better management at all levels of health services. This study is conducted to visualize the current situation of the health information system users in public health facilities in the Province of Bataan. It endeavors to answer on how the health facilities in the Province are described based on their report platform, the number of personnel, trained and untrained per facility, and the needs of the facilities when it comes to the training of the encoders. The researchers used the descriptive method specifically the Dashboarding, Analysis, and Reporting or DAR method for this study. This study focused on the data gathered from the Health Information Systems Assessment Tool. The assessment was conducted with 24 different health facilities in the Province of Bataan. The study revealed that 14 out of 24 (58%) of the Rural Health Units (RHUs) in the Province of Bataan used paper-based reporting while 10 out of 24 (42%) of the RHUs used a health information system. Twenty one out of 48 encoders (44%) are untrained while 27 out of 48 (56%) are trained. Capability training in each health information system used is proposed. In conclusion, the Province of Bataan is supporting the implementation of the use of health information systems (HIS) by designating encoders for every public health facility.

The majority of low and middle-income countries (LMICs) have frequently implemented health information systems (HIS) in response to international pressure to account for health care investments. Health information can facilitate better planning and monitoring health services, which may lead to improved health outcomes, which is the concept behind robust and effective HIS. However, the use of health information systems (HIS) as accountability tools has frequently been criticized for being counterproductive because they give health information more significance to international organizations and national governments than to those in charge of local health services. This paper aims to examine how HIS influence the development of local accountability practices and their effects on health care delivery. The technology domain of HIS is combined with a theoretical perspective derived from structuration theory. A case study of HIS in Kenya is analyzed from this point of view. The findings have implications for the application of structuration theory to comprehending accountability and the significance of IT materiality in structuration processes. It contributes to a deeper comprehension of how HIS can foster enhanced human development and health care. It also contributes to the comprehension of IS as a means of socialization through which users can negotiate multiple accountability goals as well as a means of governing people's behavior. Nurses' perception regarding barriers to use of health information system in the teaching hospital of Abbottabad

The Health Management Information System (HMIS) was designed to provide information on the status of ongoing health-related events. It has been designed to enhance data management in health care facilities. The effectiveness of this system depends on the appropriate collection, analysis, interpretation, and utilization of data. Objective: This study intended to generate knowledge to bridge this knowledge gap to overcome barriers to using and implementing integrated HIS. Methods: Descriptive cross-sectional design was used in this study. The data were collected in a Teaching hospital in Abbottabad from July 27, to Aug 3, 2022. Five item Likert scale was used to collect data from 206 nurses. Those who have one-year of experience with HMIS were included in this study. Result: More than half 51.9 % (n=107) of the study participants were female. Almost half 49.5% (n=102) of the participants responded with little and very few barriers in the use of health information systems, followed by much and very much by one-fourth of the participants 25.7% (n=53) and moderate 24.8% (n=51). technical (2.98±1.17) had the highest total mean scores followed by legal (2.95±1.19), organizational (2.91±1.15), and personal (2.90±1.16). Conclusions: The study concluded technical barrier as the top most and comprises computer shortage, internet breakdown and improper health information network. Special Section on Inclusive Digital Health: Notable Papers on Addressing Bias, Equity, and Literacy to Strengthen Health Systems.

To summarize significant research contributions on addressing bias, equity, and literacy in health delivery systems published in 2021. An extensive search using PubMed and Scopus was conducted to identify peer-reviewed articles published in 2021 that examined ways that informatics methods, approaches, and tools could address bias, equity, and literacy in health systems and care delivery processes. The selection process comprised three steps: (1) 15 candidate best papers were first selected by the two section editors; (2) external reviewers from internationally renowned research teams reviewed each candidate best paper; and (3) the final selection of three best papers was conducted by the editorial committee of the Yearbook. Selected best papers represent studies that characterized significant challenges facing biomedical informatics with respect to equity and practices that support equity and literacy in the design of health information systems. Selected papers represent the full spectrum of this year's yearbook theme. In general, papers identified in the search fell into one of the following categories: (1) descriptive accounts of algorithmic bias in medical software or machine learning approaches; (2) enabling health information systems to appropriately encode for gender identity and sex; (3) approaches to support health literacy among individuals who interact with information systems and mobile applications; and (4) approaches to engage diverse populations in the use of health information systems and the biomedical informatics workforce CONCLUSIONS: : Although the selected papers are notable, our collective efforts as a biomedical informatics community to address equity, literacy, and bias remain nascent. More work is needed to ensure health information systems are just in their use of advanced computing approaches and all persons have equal access to health care and informatics tools.

Healthcare Providers' Satisfaction with Implementation of Telemedicine in Ambulatory Care during COVID-19.

Introduction: Telemedicine has become a critical aspect of healthcare provision during the coronavirus pandemic (COVID-19). However, healthcare providers’ utilization of and satisfaction with telemedicine technologies could have a significant impact on the quality of care provided to patients during COVID-19. The current study explores the key factors that could affect healthcare providers’ satisfaction with telemedicine in ambulatory care during the pandemic. Objectives: This research study aims at identifying the factors that could influence the healthcare providers’ satisfaction level with the use of telemedicine in ambulatory care services in Saudi Arabia during COVID 19. Methods: This is a descriptive quantitative cross-sectional study. The research team has utilized the Service Quality Model (SERVQUAL) to assess the healthcare providers’ satisfaction with telemedicine in ambulatory care through a questionnaire that was adapted from previous studies. This questionnaire includes the following dimensions: tangibility, reliability, responsiveness, assurance, and empathy. It was distributed to all ambulatory care physicians in a public hospital-based ambulatory health center in Eastern Region, Saudi Arabia. Results: The study findings showed that Saudis are significantly more satisfied with telemedicine compared to non-Saudis. Age, gender, experience, medical specialty, and computer literacy skills were not found to have any significant effects on the level of the provider’s satisfaction. Conclusion: This research provides new insight and understanding of the relationship between the frequent use of the health information system and the level of physician satisfaction. This major finding puts more emphasis on the importance of education and training when it comes to the adoption of telemedicine through the frequent use of health information systems and applications. These encouraging findings provide a vital piece of information for healthcare organizations interested in a further adoption of telemedicinal practices and applications.

Health Information Systems for Older Persons in Select Government Tertiary Hospitals and Health Centers in the Philippines: Cross-sectional Study.

BackgroundThe rapid aging of the world’s population requires systems that support health facilities’ provision of integrated care at multiple levels of the health care system. The use of health information systems (HISs) at the point of care has shown positive effects on clinical processes and patient health in several settings of care.ObjectiveWe sought to describe HISs for older persons (OPs) in select government tertiary hospitals and health centers in the Philippines. Specifically, we aimed to review the existing policies and guidelines related to HISs for OPs in the country, determine the proportion of select government hospitals and health centers with existing health information specific for OPs, and describe the challenges related to HISs in select health facilities.MethodsWe utilized the data derived from the findings of the Focused Interventions for Frail Older Adults Research and Development Project (FITforFrail), a cross-sectional and ethics committee–approved study. A facility-based listing of services and human resources specific to geriatric patients was conducted in purposively sampled 27 tertiary government hospitals identified as geriatric centers and 16 health centers across all regions in the Philippines. We also reviewed the existing policies and guidelines related to HISs for OPs in the country.ResultsBased on the existing guidelines, multiple agencies were involved in the provision of services for OPs, with several records containing health information of OPs. However, there is no existing HIS specific for OPs in the country. Only 14 (52%) of the 27 hospitals and 4 (25%) of the 16 health centers conduct comprehensive geriatric assessment (CGA). All tertiary hospitals and health centers are able to maintain medical records of their patients, and almost all (26/27, 96%) hospitals and all (16/16, 100%) health centers have data on top causes of morbidity and mortality. Meanwhile, the presence of specific disease registries varied per hospitals and health centers. Challenges to HISs include the inability to update databases due to inadequately trained personnel, use of an offline facility–based HIS, an unstable internet connection, and technical issues and nonuniform reporting of categories for age group classification.ConclusionsCurrent HISs for OPs are characterized by fragmentation, multiple sources, and inaccessibility. Barriers to achieving appropriate HISs for OPs include the inability to update HISs in hospitals and health centers and a lack of standardization by age group and disease classification. Thus, we recommend a 1-person, 1-record electronic medical record system for OPs and the disaggregation and analysis across demographic and socioeconomic parameters to inform policies and programs that address the complex needs of OPs. CGA as a required routine procedure for all OPs and its integration with the existing HISs in the country are also recommended.

Time trends and social inequalities in child malnutrition: nationwide estimates from Brazil's food and nutrition surveillance system, 2009-2017.

In Brazil, national estimates of childhood malnutrition have not been updated since 2006. The use of health information systems is an important complementary data source for analysing time trends on health and nutrition. This study aimed to examine temporal trends and socio-demographic inequalities in the prevalence of malnutrition in children attending primary health care services between 2009 and 2017. Time trends study based on data from Brazil's Food and Nutrition Surveillance System. Malnutrition prevalence (stunting, wasting, overweight and double burden) was annually estimated by socio-demographic variables. Prais-Winsten regression models were used to analyse time trends. Annual percent change (APC) and 95 % CI were calculated. Primary health care services, Brazil. Children under 5 years old. In total, 15,239,753 children were included. An increase in the prevalence of overweight (APC = 3·4 %; P = 0·015) and a decline in the prevalence of wasting (-6·2 %; P = 0·002) were observed. The prevalence of stunting (-3·2 %, P = 0·359) and double burden (-1·4 %, P = 0·630) had discrete and non-significant reductions. Despite the significant reduction in the prevalence of undernutrition among children in the most vulnerable subgroups (black, conditional cash transfer's recipients and residents of poorest and less developed areas), high prevalence of stunting and wasting persist alongside a disproportionate increase in the prevalence of overweight in these groups. The observed pattern in stunting (high and persistent prevalence) and increase in overweight elucidate setbacks in advances already observed in previous periods and stresses the need for social and political strategies to address multiple forms of malnutrition.

Regulation and Use of Health Information Systems in Brazil and Abroad: Integrative Review.

There is increasing debate about the importance of using information and communication technologies to strengthen health systems. Information systems are one of the most widespread technologies for this end in different healthcare contexts. The primary objective of this study was to highlight and discuss the main characteristics of the regulation and use of health information systems in Brazil and abroad. As a secondary objective, the main characteristics identified in Brazil were compared with the findings for other countries. A robust search strategy incorporated five electronic databases. The research question was defined using the SPIDER strategy, and thematic content analysis was applied. Fourteen articles composed the final sample, and three analytic categories were identified: "Use in the Context of Health Systems"; "Implications for Health Management"; and "Communication and Interoperability." The regulation and use of health information systems in different countries was directly related to their socioeconomic context. In Latin America and Africa, policy strengthening and implementation possibilities for health management were discussed. In the United States, Europe, and Asia, the discussions on interoperability between different services was emphasized. The complexity of the Brazilian health system leads to similarities in the data analysis with several countries from different regions and with distinct political configurations.
